  • Provide the principal place of business address where your company operates.

  • Identify and appoint a Registered Agent with a physical address in Florida (no P.O. Boxes) and obtain their written consent.

  • Fill in the names and signatures of all initial Members.

  • Confirm that the LLC is Member-Managed, meaning all members are involved in business decisions and operations.

  • Customize the sections on indemnification, amendments, and dissolution if your Operating Agreement requires specific changes.
